New Directions in Diabetes Care (NDDC)
Three collaboratory classes have graduated from the 12-month diabetes collaboratory and the results were positive. This four-year initiative to meld practice redesign and clinical excellence in diabetes care has put CAFP on the national map for quality and practice improvement expertise. The fourth installment of the collaboratory (C4) focuses on creating office efficiency and capacity building for small family physician practices. We developed a modular curriculum, with particular attention to practice management issues that would assist our graduates in lowering overhead and improving provider, staff, and patient satisfaction. We launched C4 with ten former NDDC graduates with an in-person learning session in May 2009. Graduation is set for October 2010. NDDC is sponsored by The Physicians' Foundation for Health Systems Excellence, NovoNordisk, Wellpoint Foundation, PacifiCare Foundation, with in-kind support from UCSF, GlaxoSmithKline, DOCSite and Lumetra.
